Operation becomes over ratio when one out of two staff members present leaves to conduct a school pick up. One staff member is left in charge of caring for 10 children ages 3mo, 2yo, 2yo, 2yo, 2yo, 2yo, 3yo, 4yo, 4yo and 4yo.

Last fire drill conducted on 1/04/2023 was completed in 4 minutes.

One out of sinks in the children's restroom is broken.

One of four staff training records reviewed is missing transportation training. Note: This was corrected at inspection when staff member completed the training.

The walls of the children's restroom have peeling paint.

Employee providing transportation does not have a driver's license. Note: This was corrected at inspection when provider agreed to have another employee with a driver's license conducting transportation effective tomorrow.

Infant caregiver is missing trainings for training year 6/3/2020-6/2/2021 for: -Shaken Baby Syndrome -SIDS

There were 5 children in the infant classroom under the care of one caregiver. The youngest child was 5 months old and the oldest child was 2 years and 5 months old. 18 month difference between children's ages was exceeded. NOTE: This was corrected during inspection as 2 year old child was moved into another classroom.

2 school age children were observed alone in a classroom. A caregiver was in the classroom next door with the open door in between. Auditory and visual supervision could not be achieved. Caregiver was providing care for 9 children ages 2-11 years old. NOTE: This was corrected at inspection as the two children moved into the caregivers' classroom who now had a total of 11 children under her care.
