During the inspection it was observed that a child's nose was wiped while on the playground and the caregiver did not wash their hands afterwards. This was corrected at inspection when the caregiver got some hand sanitizer and used it.

During the investigation inspection it was found that a caregiver that was present during the incident only had 1/24 of their required annual training hours. This was corrected at inspection because the caregiver no longer works for Care 4 Tots.

While reviewing files at the monitoring inspection it was found that 2/3 of the files reviewed were not current on annual training hours. One caregiver only had completed 11/24 of their required hours, while the other caregiver had only completed 7/24 of their required training hours.

During the inspection it was observed that children under the age of two were having screen time.

During the investigation it was found that children under 18 months were being grouped with children up to 3 years old during the morning opening hours.

During the monitoring inspection it was found that 1/3 transportation vehicles was not equipped with the electronic child safety alarm and 1/3 of the vehicles was equipped with one, however it was not working properly.

During the monitoring inspection the emergency drill form was reviewed and the drills were not being documented accurately.

During the monitoring inspection it was found that the three first aid kits in the transportation vehicles were not equipped with tweezers, cotton balls, thermometers, and scissors.

During the monitoring inspection it was observed that a child's diaper was changed and the caregiver did not have them wash their hands afterwards. This was corrected at inspection when hand washing took place and was observed to meet compliance in other classrooms.

One caregiver who administered first aid to an injured child did not have a current certification in first aid. The caregiver is now current on first aid/CPR training.

Observed 2 children files out of 10 without the emergency medical permission completed.

It was found that a child in care was injured and the parents of the child were not notified of the injury.

It was found that medical information about a child in care was not shared with the child's caregiver which could have helped the child not get reinjured.

Observed the 18 months classroom watching television on a tablet. This was corrected when the tablet was turned off.

Observed an infant sleeping in a baby rocker. This was corrected when the infant was removed from the seat.

A caregiver who had access to the children in January until February did not have a cleared background check on file.

During an investigation it was found that an infants written feeding instructions were not followed by a caregiver.
