A toy was inspected and the hard plastic was broken on it making it a hazard for the children. NOTE: This was corrected at inspection when the toy was taken out of the playground area and thrown away.

A crib was not labeled with the child's name. NOTE: This was corrected at the inspection when caregiver labeled the crib with the childs name

Three out of four employee records reviewed for Health and Safety Audit purposes are missing hazardous materials, allergic reactions, medication administration, emergency preparedness, communicable diseases, building safety, shaken baby syndrome/ brain development/SIDS.

Childrens product certification was last reviewed on 3/5/2021.

The fire inspection was reviewed the last fire inspection was conducted on 12/27/2021. The fire inspection has expired.

During the monitoring inspection health practices were reviewed the Annual Sanitation Inspection had expired the date it was last conducted on was 2/21/21.

Required postings were reviewed the following postings were missing: Childrens Products Certification was last reviewed on 3/5/2021. Attention Parents notice was not posted for parents to review. The activity plan for each age group was not posted for the parents to review. NOTE: This was corrected at inspection when the required posting were posted on the wall for parents to view.

It was observed in the kitchen refrigerator there was an expired bottle of Mayonnaise. NOTE: This was corrected at inspection when the bottle was disposed properly.

During the inspection a ant pile of red ants was seen in the kids outdoor play area.

Most recent fire inspection conducted 2-3 weeks ago was not available for review at the time of inspection.

Binder taken during transportation routes was missing the following items: -Emergency medical transport and treatment authorization forms -Parent s names and telephone numbers and emergency telephone numbers Note: This was corrected during inspection when provider made copies of the admission forms which include the required information.

Five children's files were reviewed. Three files are missing an emergency contact, three files are missing health statements, one is missing emergency medical authorization, one is missing verification parent received policies, one is missing parent signature and one is missing emergency food allergy plan.

One out three personnel files reviewed was not available at the time of inspection.

Operation has lesson plans posted in the rooms but they are not being followed. Plans do not have the dates they are designated for and lack activities to help children meet their individual needs and developmental level.

Monthly inspections of the active play space have not been documented.

Someone at the operation tested positive for a communicable disease illness deemed notifiable by the Texas Department of State Health Services and operation did not report this information to Licensing within 48 hours. Note: This was corrected at inspection when provider provided required information to complete a self-report.

A child was observed napping on carpet. Two out the two cribs used for napping were not labeled with the children's name. Mats were observed at the operation, however, they were not labeled with the children's names nor assigned number.

Playground has overgrown weeds and grass. passageway from the center to playground was observed with trash including a metal plate with a nail and a used water bottle.

Allergy Information for a child diagnosed with food allergies is not posted in the classroom nor kitchen.

Outdoor area has overgrown grass and weeds.

Outdoor activities are not been incorporated into the infant-age children daily schedule.

Last inspection form was not posted.

Emergency Plan for a child with food allergies is missing.

Emergency Practices have not beend documented.

Operation has not developed an Orientation Training Curriculum for employees.

Lesson Plans for infant, toddler, prek and school-age children were not available for review at the time of inspection. Daily schedules do not include outdoor play during the afternoons for toddler, prek and school-age children.

Operational policies are missing Procedures for handling medical emergencies and Suspension / Expulsion Policies. Policies have to updated to reflect operation's schedule and location.

Crib's mattresses in the infant room are not specifically designed for use with the cribs' model number.

United States Consumer Product Safety Commission (CPSC) recall list has not been reviewed by operation.

Emergency Preparednesses Plan is missing a designated relocation location and lock down procedure.

Documentation that each crib meets the applicable federal rules at Title 16, Code of Federal Regulations, Parts 1219 or 1220 was not available for review at the time of inspection.

Operation has not developed an Orientation Training Curriculum for employees.

Two staff files were reviewed. Staff files contain information from a previous center and are missing: Updated employment begin date Updated Employee statement regarding center policies, personnel policies and orientation.

Personnel Policies were not available for review at the time of inspection.

First kit at the operation is missing tape, tweezers and cotton balls.

A health/sanitation inspection has not been conducted at the operation.

There is not an adult size chair or rocker in the infant room.

Four high chairs in the infant room do not have safety straps.
