Two staff files observed contained expired CPR/First-aid certification. The CPR/First aid was dated, 08/23/2023, and was valid for two years.

The childcare facility has not practiced, and/or documented emergency practices, including the fire drill log. The school-age program began care in August of 2023. As of September of 2023, there was no documentation related to fire drill practices.

One staff CPR/First-Aid certification observed did not adhere to the guidelines of skill-based training or consist of a curriculum that incudes use of a CPR manikin.

Two out five employee files did not contain a signed and dated statement for orientation, and/or pre-service.

During the annual monitor inspection, the newly appointed Director had not submitted a Governing Body/Director Designation form to Licensing. Licensing learned during the inspection of the new Site Director position for this location.

One out of five staff training records were not avaible for review. The most recent training observed included trainings from 2020.

Two out of five staff files observed did not include a notarized affidavit. One form was signed by the individual, or employee, but was not notarized.

Three out five staff records did not include a start date for the employee. A personnel form was not avaible for review in order to reflect the employees start date. One staff added their employment date while Licensing was present.

The operation did not have a food menu/snack menu available for review. The Site Director was able to document a snack list for review.

Three staff training certificates/records were not available for review. Records were being observed for the purposes of a Health and Safety Audit. Two staff/employees were able to provide their phone to view the training certificates.
