During a walkthrough observed, the fire inspection expired on 1/2/24.

The provider's last review was on 7/1/23 for the Children Product Certification form. This was corrected when the provider reviewed and signed the form at the inspection.

During the inspection it was found that the sanitation inspection was overdue by one month.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

Observed paint peeling from the window ledges and growing thorny grasses on the playground.

During the follow-up inspection it was observed that the grass was overgrown on the playground, about 2 feet high.

During the initial inspection it was observed that a caregiver changed 4 children's diapers, one after another, and did not sanitize the changing table pad between each use.

During the initial inspection it was observed that a caregiver did not have the children wash their hands after their diaper was changed.

During the initial inspection a caregiver was observed changing four children's diapers, one after another, without washing their hands or changing out their gloves between each diaper change.

During the 3rd initial inspection a 4 month old infant was observed sleeping in a restrictive device. a bouncer, which was placed into a crib.

During the monitoring inspection it was observed that the children ages 16 months - 23 months were served hot dogs that had only been cut in half, still circular in shape.

3 out of the 5 files reviewed did not have health statement forms, from parents or pediatricians, on file.

During the 2nd initial inspection the trasnportation vehcile was reviewed and found that the electronic safety alarm was not functioning properly, and was not installed in the back of the van.

During the monitoring inspection it was observed that there was no loose fill under the spring rockers or at the bottom of the slide in the playground area.

During the 1st initial inspection it was observed that the operation did not have the children's product annual certification completed.

During the 1st initial inspection it was observed that two caregivers arrived at the operation and did not wash their hands after arriving.

During the 1st initial inspection postings were reviewed and the operation did not have an activity plan for each group of children in the child-care center.

During the 1st initial monitoring inspection it was observed that the operation was over ratio on two occasions. During opening hours the specified age group was 2 years old and the operation was in a 1/12 ratio, and should have been 1/11. Later into the inspection a caregiver was with infants in a 1/6 ratio and should have not had more than 5 infants without a 2nd caregiver present.

During the 1st initial inspection it was found that the operation had not been conducting their daily grounds and safety checklist.

During the 1st initial monitoring inspection it was found that the operation had not conducted a fire drill since their first opening day May 15, 2023. This was corrected at inspection when a fire drill was conducted and documented.

During the 1st initial inspection postings were reviewed and the operation did not have a current employee list posted. This was corrected at inspection when an employee list was posted.
