A wheel was missing on the toddler playground equipment which caused a protruding piece of metal to be in the way of the play area. Tree roots were exposed on the toddler playground, which could be a tripping hazard.

This standard was evaluated as part of a DFPS investigation and found to be deficient. A child was pinched on the arm by a caregiver as a form of discipline.

The chain link fence for the large playground had three portions that were not connected to the bottom rail and were loose from the side posts. The integrity of the fence is compromised allowing exits points for the children.
