One child's medication was expired.

The operation did not have a current sanitation inspection.

Information obtained throughout the investigation shows that a teacher failed to supervise children when two children left the classroom and went outside. The children were unsupervised for approximately 1 minute and 15 seconds.

Information obtained throughout the investigation shows that the operation failed to obtain a parent signature on an incident report within 48 hours of when the incident occured.

A classroom had 21 children between 3-4 years old with one caregiver.

A caregiver stated she had 13 children present when she actually had 14.

One child was missing their 3rd and 4th dose of Dtap, IPV, PCV, and Hib vaccines. Another child was missing their 1st dose of Hep A, and the 4th dose of Dtap and PCV immunizations.

The children's files reviewed were lacking if water activity assistance is required.

There were no substitutions recorded for the lunch served on 03/20/2024. This was corrected at the inspection when the director created a substitution form for the meals not served.

During the inspection the last documented sanitation inspection was in 2020.

One diaper changing pad was observed with a tear that prevented it from being properly sanitized. This was corrected at inspection when the director replaced the pad.
