Based on the evidence obtained, there is sufficient evidence of a child being placed in the restroom with the lights turned off in response to a child crying.

In a crib where an infant was laying, contained a milk bottle with milk inside. This was corrected at inspection once the milk bottle was removed from the crib.

The refrigerators and freezers had dried food and crumbs on the shelves and where food was stored.

Five personnel files reviewed did not have a notarized affidavit on file for review.

During the inspection there was no documentation of carbon monoxide detectors being tested. This was fixed when the carbon monoxide was tested.

While reviewing files there were two employees without preservice training on file.

During the inspection there was a known child with a food allery and no posted emergency plan. This was fixed when the plan was posted.

There was a few boards in the fence line that were not touching the ground that created a gap in the fencing.

There was no action plan on file for a child that has a known food allergy.

The first aid kit located in the vehicle that transports children, did not have scissors or a thermometer, to make the kit complete.

There was less then 6 inches deep of loose fill underneath and around a structure meant for climbing, the ground was visible in some areas around the climbing structure.
