One out of the five employee records reviewed for health and safety audit was missing the following required trainings hazardous materials, medication administration, and building safety for the annual year 5/20/2024-5/20/2025.

Director is missing the following required trainings hazardous materials, building safety, emergency preparedness, allergic reactions, medication administration for the annual year 8/17/2023-8/17/2024.

Two of the employee records reviewed were missing the following trainings hazardous materials, allergic reactions, building safety, communicable diseases, and emergency preparedness required annually.

Staff files were reviewed, one staff file was missing the education qualifications.

A 10 month old was watching a video on the TV with shapes and pumpkins. NOTE: This was corrected at inspection when caregiver turned off the TV.

A child was on a high chair watching a video about marbles, the video did not follow with the planned activities.

CPSC Certification Form was last reviewed/signed on 01/04/2022.

January and February 2023 fire drills were conducted but both drills took over 3 minutes. No lock down drill were conducted during 2022. Two severe weather drills were conducted during 2022.

An prospective employee was present at operation and interacting with children prior to operation receiving background check eligibility notice. Note: This corrected at inspection when prospective employee left operation.

The lid to the diaper pail in the infant room could not be closed as it was full. The trash can in one of the restroom does not have a lid and used diapers were observed inside the trash can. Note: This was corrected at inspection when provider emptied the diaper pail and added lids to the trash cans.

Carbon monoxide detector was last checked on 11/17/2022. Fire Extinguishers were last checked on 02/08/2023 but this was not documented. Note: Provider checked CO detector and documented the checks for fire extinguisher and CO detector during the inspection.

A sink and toilet designated for children use has been out of service for a month.

An infant age child was asleep in a rocker and covered with a blanket.

The following items are not being taken during transportation route: -List of children being transported -Emergency medical transport and treatment authorization forms for each child being transported -Parent s names and telephone numbers and emergency telephone numbers for each child being transported

There is not a hand-washing sink within young infant classroom.

Four children's records were reviewed. One record was not available for review at the time of inspection. A second file is missing admission date, emergency contact information, release information, permission to transport / field trips / water activities, health care information, emergency care authorization, special care statement and school information.

No carbon monoxide detectors and fire extinguisher checks have been documented for 2022.

An infant-age child was a sleep in a play-yard without a sheet.

Most recent sanitation inspection report could not be located at the time of inspection.

Feeding instruction for a child not yet eating food were last reviewed / signed on 3/16/2022.

Older infants classroom is between Prek and School-age room. Children must cross by infant room to access school-age room and there is restroom designed for children in the infant room.

Operation started offering Transportation services a month ago. CCR was not notified. Note: This was corrected at inspection when provider made inspector aware and the transportation subchapters was evaluated.

Facility needs to designated an individual that meets director's qualifications and its present at the day care center during business hours.

Director is not present during business hours and is not able to supervise employees or carry out director's responsibilities.

Facility is not current with Health Inspection.
