After conducting interviews, it was determined that a caregiver brought a gun on the premises.

This standard was evaluated as part of a DFPS Investigation and found to be deficient. Prohibited punishment of grabbing a child in care was displayed by a caregiver.

New Admission forms did not include the required emergency consent for treatment.

This standard was assessed during an investigation. A caregiver grabbed two children by their arms.

It was determined that the operation had ants inside of the classrooms. However, the ants were later treated.

One child did not have the required Food Allergy Plan for known nut allergy.

The operation knowingly allowed persons to be present at the operation before receiving notification from the CBCU that the persons eligibility determination is Eligible, Provisional or Eligible with Conditions.

An infant was allowed to sleep in a restrictive device (bouncer) during the walkthrough.

A Royce City ISD staff member was present at the operation before receiving notice from the CBCU regarding the person's eligibility. The employee had been fingerprinted through the ISD.

The infant staff were not using the written feeding instructions signed every 30 days by the parent or physician.

There were no immunization records for one of the six chilren's records reviewed.

There were 2 children's records of the 6 which were reviewed which did not have a health statement as required. You must ensure all children have a health statement on file that they may attend child care. A health statement is: (1) A written statement, from a health-care professional who has examined the child within the past year, indicating the child is able to take part in the child-care program; (2) A signed affidavit from the parent stating that medical diagnosis and treatment conflict with the tenets and practices of a recognized religious organization of which the parent is an adherent or a member; or (3) A signed statement from the parent giving the name and address of a health-care professional who has examined the child within the past year stating that the child is able to participate in the program. This must be followed by a signed statement from a health-care professional within 12 months of the date of admission. You must have a health statement on file at the center, within one week after the date of admission, for each child who does not attend pre-kindergarten or school away from the child-care center.

There was no verification of liability insurance available at the operation during the inspection.
