There was sufficient evidence found during the investigation to show that adequate supervision was not provided when a child was able to walk out of the front door of the operation.

A situation placing a child at risk was not reported to child care regulation within the required timeframe.

It was observed that the operation was using a community insect repellent without written parent permission.

One bucket swing was observed on the outdoor playground. There was no protective surfacing observed in the use zone of the swing.

It was observed that one staff member did not wash their hands before the next activity after a diaper change.

Six of eight children seated at a raised feeding table did not have the safety straps buckled on their seats. The children were buckled during the inspection.

One caregiver was observed to walk away from the diaper changing table, leaving a child to retrieve diapering supplies.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

There were two caregivers who were unsure of who exactly they had during the walkthrough as far as how many children and their ages.

There was a caregiver who did not have current First Aid and CPR certification.

Written infant feeding instructions had not been updated since January 2023.

There were seven children in care who did not have immunization records on file.

An infant was observed swaddled in a bouncer and had just fallen sleep.

During the inspection, children were left alone while eating. Children were left alone in the classroom, while their caregiver went to the office.

There was one caregiver with eleven infants from ages 4 months to 16 months.

One television was not anchored.

There was one caregiver with a pending background check supervising children, and two caregivers supervising children with no background checks.

There was a unsafe structure in the outside play area that poses a risk to children.

There were no sinks in two of the diaper changing areas.

The health inspection has not been completed.
