While reviewing training files for the HASA, 2 employees did not have the required 24 hours complete.

The operation changed their operating hours without notification to CCR.

There was one caregiver caring for a group of children unsupervised that had a provisional background. This was corrected when the operation put another caregiver in the room.

This rule was reviewed in relation to the reported allegations and found to be deficient. The DFPS workers are the legal guardians of a child at the operation and were not allowed back to visit the child's classroom when they requested. This is a violation of the rule.

This standard was found deficient during the DFPS investigation. The infant children were left unsupervised in the classroom when the caregiver left the room to take a school age child to their classroom.

This standard was reviewed in relation to the reported allegations and found to be deficient. Three children exited the building from a door in the infant room and were outside of the building unsupervised for over 10 minutes.

This standard was found to be deficient during the DFPS investigation. The incident reports for the incident were not signed by the parent and one parent did not receive an incident report.

This standard was found deficient during the DFPS investigation. The operation allowed a child that was dropped off by the school bus to come in the door from the outside directly into the infant room and pass through the room and then be taken to their room.

A child was observed asleep in a bouncer seat during inspetion at nap. The child removed during inspection and placed on a mat. There was not a mat out for the child during the initial walk through.

During the opening time the caregiver had 3 infants and 8 older children for a total of 11 children, leaving her out of ratio. This was corrected at inspection when the director arrived.

During inspection the baseboard in the infant room was missing and the broken sheetrock was exposed within the reach of the crawling infants. This was discussed with the operation.

During inspection the van was observed to not have a safety alarm on the vehicle as required.

During inspection the two year old room was observed to not have a hand washing sink in the diaper changing area for children over the age of 18 months old. There was not a sink for the caregiver to wash hands either after a diaper change.
