At time of inspection, operation did not have documentation showing the rating for shock data testing on unitary surface for playground.

At time of inspection, the unitary surfacing on playground has not been maintained. There are missing tiles and some tiles have been lifted and moved.

Based upon the investigation conducted the operation does not have documentation for accommodations from a health care professional for children with known special needs.

Fire extinguishers had not been inspected monthly by director or designee.

Carbon Monoixde detectors had not been tested monthly by director and or designee.

Smoke detectors had not been tested by director and or desginee.

Emergency drills for August and September of 2024 had not been documented.

Operational Policies did not state how operation will support special needs services for children in their care.

The fire inspection has not been conducted within the past 12 months. The last fire inspection was conducted 9/3/22.

The monthly infant feeding instructions have not been updated for 3 out of 5 infants present today. The last updates were dated between August 2023 and September 2023.

A special needs teenager was in the school age room alone.

In the older infant room, there is not a sink in the diaper changing area.

There was an infant sleeping in a crib with sheets that appear to be stained or soiled.

There were cleaning supplies accessible to children in the cabinet below the sink in the inclusion classroom's bathroom. The hallway mop closet is accessible to children because it does not lock.

The unitary surfacing located on the playground was observed to be unkept.

Children were observed resting without the required mats.

One of ten student records did not contain the elementary school phone number.

Two infants did not have the required feeding instructions.

One first aid kit did not contain cotton balls. SN: This was corrected during inspection.

The twos classroom was observed to have students sleeping without sufficient walking space.

In the school age room there was a cable outlet plate that was completely unscrewed from the wall exposing a whole and the dry wall. There were several paint chips in the after school room, 2 year old room, and the toddler room. Door frames and doors had visible dirt and grime on them. At the end of the building, closest to where the new construction is being done, the fence is missing. There are several boards along the fence line that are broken or pushed away from the fence. Three out of the four caregiver files reviewed did not have the training listed under this standard.

Three children files out of eight reviewed did not have a copy of the child's immunization record.

The changing mats in the infant room and toddler room were ripped leaving the foam padding exposed and unable to be sanitized properly. Note: Pictures were taken for the file.

There is a retention pond behind the parking lot and near where the school age children play. The pond is currently full of water due to recent rain.

The caregiver did not display good judgement when driving the school age children around on the daycare bus, by not stopping the bus when the children were standing, jumping and fighting.
