There were 3 caregivers present at the operation who did not have background checks. They left the operation.

Two classes combined for naptime and stayed that way the remainder of the day on 6/12/2024. Five children ages 3 to 5 years old were mixed with 9 children ages 14 months to 21 months old.

The director left her job suddenly and did not appoint someone to be in charge after she was gone for the day.

All four caregivers working on May 17th report that a child did not have their temperature taken even after being asked by the teachers and parent. By the time the parent picked up, the child's temperature was 104.

There were four infants without signed and dated infant feeding schedules.

There was not an allergy action plan signed by a doctor for a child who has an Epi-Pen.

The director did not her training record available for review during inspection.

There was no a carbon monoxide detector that was installed at the time of the inspection.

The fire extinguishers were last serviced in October of 2022.
