Three children in care stated they were spanked by foster parents. The foster parents admitting to spanking one foster child on the hand using their hand and spanking the other two children on their hands/bottoms using their hand or a rubber spatula.

During the inspection, it was discovered a child was almost 60 days late having a TB test completed.

During the walk-through medication was discovered in the master bathroom drawer, The drawer was not locked, nor was the bedroom or bathroom door locked.

During the inspection, interviews confirmed medications are occassionally administered by an untrained and unqualified household member.

During the inspection, I confirmed the presence of live bedbugs in the foster child's bed.

Out of the 4 foster home files reviewed one foster home file's fire inspection did not have the phone number of person who conducted the inspection.

Out of the 4 foster home files reviewed one foster home file did not have a second fire extinquisher. This foster home is a 2 story home.

During the sampling inspection, the trampoline was observed as not having the required shock absorbing pads over the springs and hooks.

Ms.Washington was unable to locate her verification certificate during her inspection and visit with the home members.

During the sampling inspection, the playground equipment was observed to be not well maintained and needing repair. There was missing wood and protruding nails.

During the sampling inspection, it was discovered the neighbors babysit the child in care but has not completed a background check with the agency.

During the sampling inspection, there were several outlets throughout the home that were not protected with outlet covers.

During a review of a child's record, a file labeled March over the counter medication was actually a daily note log.

Two of the four records reviewed indicated the two children were prescribed a daily medication, however, medication logs indicated "none at this time." There is no evidence on medication logs supporting that medication was administered daily. One child record indicated a child was prescribed a daily supplement, however, there is only one record indicating that the child was administered the medication. The one child record does not show that this supplement was discontinued.

One of the four child records reviewed did not have child/parent signatures indicating child rights had been reviewed within seven days of the child's admission.

One of the four records failed to provide notification to the foster parents.

One of the four child records had a Preliminary Service Plan that was thirteen days late.

Both child discharge summaries did not include the current medication at the time of discharge. The summary indicated the medication regimen as n/a, however, the medical forms for both children show they are currently taking a prescribed medication.

One of the four employee records reviewed had a Psychotropic Medication training that was two months late.

One of the four staff files reviewed had a Licensing Affidavit for Applicants for Employment form that was not notarized.

In all four records reviewed, the fire/severe weather drills lacked documentation of the start and end time of the drills.

In all four records reviewed, the home study did not specify that 60-days or two consecutive calendar months were collected to verify the family's monthly income.

The agency failed to report the quarterly EBI data timely.

1 out of 6 children's records reviewed had a late 72 hour preliminary service plan.

6 out of 6 children records reviewed were missing contact information between child placement staff and child. The length of contact was not mentioned in any of the records.

One caregiver stated that a child in care has been told that if the child in care hit a child, the caregiver would hit the child in care. Both the child in care and caregiver reported that the caregiver has a spoon "Stormie," that was used to discipline the biological children in the home. The child in care reported being spanked with "Stormie," by the caregiver.

The child in care, biological child, and caregiver stated that the caregiver popped the child on the buttock in the store using the caregiver's hand.

There were two medications out in the kitchen that were not in the locked box with the other medication.

The caregiver admitted to locking the bedroom door of a child in care to keep the child in the room in order to conduct household tasks.

One home study did not include a monthly itemized household expense report

Fire and Health inspection evaluation expired 08/19/2020 in one home record. The inspections weren t completed until 10/08/2020. Another home record fire inspection expired 09/06/2020, and the inspection wasn t completed until 09/14/2020

At the time of the inspection, chemicals were accessible to a child in care.

The caregivers have been spanking the girls in the home (now in the adoption unit) with a wooden spoon.

The caregivers interfered with the investigation by making misleading statements to DFPS staff.

A 3-year-old child was inappropriately supervised, resulting in the child overdosing on prescriptive medication

The caregiver failed to lock/securely store a child's ADHD medication, resulting in a 3 year old child in care accidentally overdosing.

A caregiver did not return medication to secure storage allowing accessibility to a resident. Medication was ingested and required emergency medical intervention.
