There was not a hand washing sink or diaper changing station in the classroom with a child who is 17 months old.

The playground maintenance checks and building grounds were not documented for the previous two weeks. This was corrected at inspection when the caregiver filled out the days that the playground maintenance checks were conducted.

Operation was caring for infants without prior approval. Licensing did not approve operation to care for children 17 months and younger without the proper equipment being added.

There was not a rocking chair in the classroom with a child who was 17 months old.

One child does not have a health statement or parent signature verifying that they have been examined by a health care professional in the past year.

One caregiver's shaken baby syndrome, SIDS, and early childhood development training certificates were unavailable for review.

Three ceiling tiles in the preschool classroom are damaged and in need of repair or replacement. (two tiles have holes in them and one is warped and has exposed insulation). There is a drain cover in the floor of the preschool classroom that has a soft spot & needs to be reinforced & the surrouncin tiles replaced or repaired.

Caregivers did not have current training certificates available for review for the required training topic at the time of inspection.

Out of two drivers, one was not current on the required annual training.

The operation did not have a record of a pest extermination available for review at the time of inspection.

One children with a documented medically diagnosed food allergy did not have a plan of action if the child ingested the food it was allergic to or a parent signature.
