Four of the ten children's files reviewed were missing the signed health statement form the physician.

Two of the ten child files reviewed were missing current immunizations.

Four fence pickets on the outside playground were either missing or hanging onto the playground causing a hazard.

A panel was missing form the outside play area fence.

The loose fill on the climbing structure was not at the proper depth of 6 inches.

Four staff members do not have valid documentation showing they meet the educational requirements for a caregiver. *Copies were obtained at the time of inspection. 11/23/2021 - The violation has been re-cited. Only one staff members provided the information required.

The two play kitchens on the playground are heavily soiled and need to be cleaned. The fenced are on the concrete patio area which stores toys and outdoor equipment is in need of clean and the mulch bag needs to be relocated off out of the area. Photographs were taken at the time of inspection.

The parent handbook does not include a section for Vaccine Preventable Diseased in regards to staff member.

The posted activity plan in the preschool classroom does not reflect moderate and vigorous activities for a total of 90 minutes per day.
