During inspection, fire extinguisher in one of the van's was observed not secured.

During investigation, it was determined that a child's written feeding instructions for an infant not yet on table food were not followed correctly.

During investigation, it was found operational policies regarding diapering and potty training did not mention a restriction on caregivers entering the bathroom. Note: This was corrected at inspection when operational policies for diaper and potty training were updated to include the information.

A fire inspection was found outdated, last inspection was dated 7/13/2022.

The operational policies were missing the information pertaining to physical activities.

A staff member's purse in the four-year-old classroom was sitting on a shelf accessible to the children in care. Note: This was corrected at the inspection. Five classrooms were noted with electrical cords hanging and accessible to children. Note: Extension cords were secured at the time of the inspection.

Three infants (2-4 month old) were found not sleeping on their backs.

It was noted that a caregiver in the three year old classroom grabbed a child's chin as a form of discipline while redirected the child.

Six children's heads were covered during nap time. This was corrected when the children's heads were uncovered.

At the time of inspection the mulch on the larger playground was not maintained at its proper depth.

At the time of inspection the caregiver in the two year old classroom did not wash their hands after diaper change.

At the time of inspection one infant under 12 months was sleeping in a swing. This was corrected when the child was placed in the crib to nap.

The diaper changing mat was not properly sanitized after a diaper change.

Two slides did not have the required playground surfacing in place. This was corrected when the slides were removed from the playground.

At the time of inspection the caregiver in the two year old classroom did not wash a child's hands after a diaper change. This was corrected when the child's hands were washed.

At the time of inspection one caregiver in the two year old group left the second caregiver alone with 12 children in the gym. This was corrected when the caregiver returned to the group.

At the time of inspection five children's files evaluated did not contain a special needs statement.

Two children were not strapped in to the high table chair. This was corrected when the children were strapped into the high table chair.

At the time of the inspection there was not an adult size chair in the older infant room. This was corrected when the adult size chair was placed in the infant room.

There were two boards on the fence around the after school playground that were broken.

The following conditions were not being met during the inspection: New employees hired during the probation period have a week to be notified of the conditions which requires signed confirmation. Signed confirmation for 2 new employees hired on 2/8/21 and 2/4/21 had not been completed. The director must complete weekly observations for a minimum of 30 minutes for each classroom. Observations for the week of 2/22/21 were not available for review. The director must meet with all staff following an inspection to review deficiencies cited and must have signed acknowledgement on the inspection form. 1 staff member's signature was observed to be missing on the operation's most recent inspection form.

Playground equipment on the pre-k playground was observed to be cracked, creating a sharp/rough edge which created a hazard.
