A 15-month-old infant was observed in the outdoor play area, where they picked up a rock and brought it to their mouth, highlighting a potential safety concern.

The six children present at the operation were not signed in. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

The director did not have a current transportation training.

The activity plan in one classroom did not have a date and another activity plan posted in the classroom was not current.

Last Fire Drill was conducted on 9-13-24.

Expired food items were found in the fridge including milk, carrots, bread and tortillas. Note: This was corrected at inspection when provider threw away the expired items.

Last fire drill was documented 1/9/2024 and there are no documented CO detector nor fire extinguisher checks. Per provider; drills and checks are completed monthly but were not documented. Note: This was corrected at inspection when CO detector and fire extinguisher were checked and last fire drill were documented under the emergency practices form.

A review of the Employees Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.
