There were diaper creams and sunscreen in a classroom that were not labeled with the child's name. This was corrected at the inspection when the caregiver labeled each cream and sunscreen.

A class with children younger than 5 years old had 2 safety outlets that did not have safety plugs in either the top or bottom plug. This was corrected at the inspection when the caregiver put safety plugs in all uncovered outlets.

Children's personal storage was not labeled with the child's name or picture.

The bus driver did not a valid training certificate for transportation training.

The playground had equipment and chairs turned over, a door to a storage closet was off the hinges, and a gate was broken causing splintering wood.

The cots were not labeled with the child's name nor was there a nap chart with a number assigned to each child.

The classrooms and hallways were to dark during nap time. This was corrected at the inspection when each caregiver removed the blanket from the window or opened the blinds and the director turned on the light on in the hallway.
