A caregiver caring for a group of children did not complete the required 24 clock hours of pre-service training within 90 days of employment. The caregiver was missing 11 clock hours of pre-service training.

A caregiver caring for a group of children did not have current training in pediatric first aid with rescue breathing and choking. The training certificate available for review was for training completed in Standard First Aid.

A caregiver caring for a group of children completed the pediatric CPR training online.

The Emergency medical transport and treatment authorization forms and Parent's names and telephone numbers and emergency telephone numbers were missing from the transportation vehicle binder for two children being transported. Note: This was corrected during the inspection when copies of the required forms were obtained and placed in the transportation vehicle binders.

A person who is provisionally eligible to be present at the operation with conditions was performing duties that violated a condition placed by the Centralized Background Check Unit.

One out of four staff training records reviewed is missing trainings on the following topics: Hazardous Materials Food Allergies Administering Medications Emergency Preparedness Communicable Diseases

Four staff files were reviewed. One file is missing copy of high school diploma/ged or equivalent and a second file is missing 16 hours of pre-service training.

There are cleaning products acceissble to children underneath the sink of the children's restroom. Changing mats in the infant and toddler room have tears.

One out of four staff training records reviewed has a first and/CPR training that expired 9/14/2021.

Two out four children's files review were not available for review at the time of inspection.

Two exit doors in the operation were locked.

An infant was observed asleep on a bouncer/craddle.

Director did not have the following annual required training topics: (1)Recognizing and preventing shaken baby syndrome and abusive head trauma ; (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS); and (3)Understanding early childhood brain development.
