A child in care who had a listed diagnosed food allergy did not have a complete food allergy in file when reviewed as it was missing the doctor signature.

The background check validation list has not been validated and there were individuals listed that have been terminated from the operation for abouth a month, which is longer than seven days of them leaving.

Based on the evidence obtained during the investigation, the staff were unaware of the on-going activities and lacked auditory and visual awareness of a child they were responsible for when the child left the classroom to the outdoor space area. Additionally, the child was discovered by another staff member whom made the caregivers responsible for this child, aware that the child was missing.

In two classrooms at the operation, bottles of bleach spray are in cabinets that are accessible to children.

Four of the ten children's records reviewed did not have a completed section regarding the name, address, or phone number of their physician or emergency care facility.

Based on the evidence obtained during the investigation, the operation has been using a building that has not been approved or permitted by Child Care Regulation for staff functions/trainings and for children to participate in physical activities when the weather is cold outside.

Based on the evidence obtained during the investigation, a caregiver did not supervise a child when the child left the classroom and into the main hallway as the caregiver did not have auditory or visual awareness and also was not aware of the child's activities.

The operation did not notify Licensing of a communicable disease within the required two day timeframe.
