The multi child tricycle ride located on the playground does not have the required 6 feet of use zone between it and the concrete sidewalk.

The merry go round that requires 6 inches of loose fill in the fall zone has less than an inch of loose fill. Additionally, the anchoring devices of the merry go round are exposed and not covered due to the lack of loose fill.

The director did not have current CPR and First Aid training as it expired on 2/2023.

Initially, during this inspection, the director was not present and all the staff were not aware that director was not present.

An employee that has not been employed for two week was still listed on the operation's employee list. **The director corrected this at inspection when the individual was inactivated using the provider portal account.

A child file was missing the information for the emergency medical authorization section for a child enrolled in care. Additionally, this child was missing a parent signature for the health statement information section.

Four staff files were reviewed. One staff file was missing a high school diploma and a photo identification. Three staff files were missing the affidavit of employment. **The operation corrected the photo identification that was not in file when the employee's driver's license was copied and a copy place in the file. The director is also a notarizer and completed one affidavit of employment during inspection but two continue to be incomplete at this time.

One child's file was reviewed and did not have any immunizations in file. *This was corrected at inspection by the director and it is in file.
