One out of four staff training records reviewed is missing trainings on the following topics: -SIDS / Shaken Baby -Food Allergies -Administering Medications -Communicable Diseases

Operational policies were reviewed and are missing / need to updates under the following items: -Days of operation -Policy regarding illness and exclusion criteria -Suspension / Expulsion policy -Meals provided -Immunization Records requirements -Enrollment Procedures -Physical / Outdoor Activity policy including duration, type, setting, recommended footwear and clothing and extreme weather criteria -Inclusive Care Services

Administering Medications and Food Allergies trainings were completed and presented by the same person on the date/time.

One out of four staff files reviewed was not available at the time of inspection. A second file is missing TB test and statements regarding orientation, personnel and operational policies. A third file missing statements regarding orientation, personnel and operational policies.

Lesson plans posted were from December and there were no specific planned activities for the day.

Three out of four children's records reviewed are missing up to date immunization records. One out of four children's records reviewed is missing child s physician or anemergency-care facility information.

Fire drill conducted on 01/016/2023 took 5 minutes.

Three out of four staff training records reviewed are missing trainings on communicable diseases, food allergies and shaken baby / safe sleep.

Children's Product Safety Recall List was last reviewed and certified on 09/03/2020.

Binder taken during transportation routes is missing the emergency medical transport and treatment authorization forms for each child being transported. Transportation vehicle is missing center s name and telephone number clearly visible on the outside of the vehicle.

Immunization records were not available for review for 1 out of 4 files reviewed during inspection.

Owner reported there was a flea infestation at the operation at end of March 2021. This was not notified to Child Care Regulation timely. NOTE: Fumigation of building and outdoor area were completed by a Pest control company on 3/24/21, 4/10/21, 4/26/21. Receipts were observed. Owner is completing monthly fumigation.

Emergency lighting or battery-powered lighting was not present in the four classrooms at the operation.
