A child in care did not have an up to date immunization record.

A child in care with a diganosed food allergy did not have a food allergy plan on file at the operation.

During an abuse neglect investigation inspection there were 38 pre-k and school age children with 2 caregivers.

It was determined through the course of the investigation that the operation failed to report a situation that placed a child at risk when a child was left outside on the playground unsupervised.

It was determined through the course of the investigation that a child was left unsupervised on the playground.

Upon arrival the director was not present. The caregiver's at the center, when asked, gave different names of persons in charge. This was corrected when the director arrived at the operation and director responsibilities were discussed.

A child's record was lacking a health statement and immunization record.

7 caregiver files that were reviewed did not have valid CPR training.

It was determined through the DFPS investigation that a school aged child fell asleep during transport, was not accounted for at unloading and was left unattended on the daycare van for approximately 20 minutes.

It was determined through the DFPS investigation that a child was neglected while at the operation.

It was determined a toddler was left unattended and unsupervised on the play yard for 10-15 minutes.

Children's hands were not being washed after changing of diapers/pull ups.

It was determined a caregiver did not properly maintain visual and auditory awareness of child that had gone to the bathroom. The caregiver is no longer employed at the operation.

A caregiver was changing pull ups/diapers in the classroom and did not have access to a sink for handwashing.

The caregiver admitted a child in care was left outside unsupervised for approximately 3 minutes when the rest of the group went inside for lunch. This was corrected when the child knocked on the door and was let inside by staff.

There were 3 different school age children observed entering the building and getting water bottles, and going to the bathroom unsupervised while the rest of the children and the caregiver were outside on the playground. This was corrected at the inspection when a another caregiver was positioned to oversee any child entering the building.
