Two out of four children observed seated at the table with bucket type seats were observed to not be strapped properly in their seats.

In the older children's outside play area, the underlining black material beneath the tire mulch surfacing, was exposed in several places posing tripping hazards.

Two volunteers were present at the operation and providing care to children without prior background check eligibility notice. Note: this was corrected at inspection when volunteers left operation and provider indicated they will not return until background eligibility is received.

Four out ten staff training records reviewed were missing certificates for health / safety related topics and eight out of ten files were missing up to date first aid / CPR training.

Operation consist of 3 separate buildings. The 18mo and school-age building is missing a carbon monoxide detector.

Play structures in the toddler and prek/school-age playground are not meeting the required 6ft use zones.

Loose PVC pipe was observed to be on the ground causing a potential tripping hazard on the 18 month playground.

A review of the Employee Who Left Employment in 2023 data from January 18, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

Of the three Infant files reviewed, all three did not have the Infant Feeding Instructions updated.

A review of the Employees Who Left Employment in 2022 data from January 26, 2023, revealed that the center had not reported the number of employees who left employment in 2022, as required.

Employee files evaluated did not have an original certificate or letter documenting the completion of required trainings pertaining to the health and safety topics.
