The operation did not have the test data information for their unitary surfacing materials for their large playground and their playground on the patio.

This standard was tasked over for this investigation and found to be deficient. It was determined during the investigation inspection that the operation did not report the allegations made to Licensing in a timely manner, which is at most within a 48 hour timeframe.

This standard was tasked over for this investigation and found to be deficient. Upon review of the investigation footage, the caregiver present was observed pulling a child in their chair away from the table area in an aggressive manner.

This standard was tasked over for this investigation and found to be deficient. Upon review of the investigation footage, a caregiver was observed pulling a child away from their table in their chair and placed him away from the group. The child remained strapped in their child in this area for over 5 minutes.

This standard was tasked over for the investigation and found to be deficient. After further review of the investigation footage, it was determined that the caregiver present did not interact positively with all the children present at the time. The caregiver did not speak to every child, attend to, or engage with each one to ensure he or she had what they needed at the time.

The operation unitary surfacing has not been maintained as instructed.

The operation failed to administer medication as stated on the label of directions. Administered on 8/2/21 and 8/3/21.

I observed 5 cribs that held a variety of additional foam or pads under the mattresses.
