At the time of the inspection a calendar was observed with a field trip schedule posted for the summer of 2024. It was found that the operaiton had conducted field trips the previous summer, but not for the current calendar year.

At the time of the inspection it was found that only 5 fire drills were logged for the current calendar year.

At the time of the inspection a caregiver left the room while children were sleeping without notifying the person in charge that they needed to step out. The room was within a distance and open that auditory awareness was able to be maintained.

At the time of the inspection the playground area was observed with the fence bowed out, leaving the pointed bottom area of the fence out where it could easily scratch a child. There were also broken toys observed laying around the area. There was animal feces observed in a few spaces around the play area.

At the time of the inspection the operation had not updated their operational policies to include information regarding Parent's Rights.

10 children's records were reviewed. Various items were missing or outdated. Items were reviewed with the director.

At the time of the inspection the playground area had one exit tied shut with a white rope and a chain wrapped around both poles securing it closed and clipped in place.

At the time of the inspection a vehicle used for transportation did not have the required fire extinguisher present.

At the time of the inspection it was reported by staff that when all children enrolled for after school care are picked up 2 children will have to share a seatbelt in the back seat.

At the time of the inspection the vehicle used for transportation did not have a first aid kit present.

At the time of the inspection children that would require a booster seat in a general vehicle were transported without a booster seat in the vehicle.

At the time of the inspection 1 employee did not have the required affadavit notarized and on file.

At the time of the inspection 4 staff did not have documentation to show their level of education on file.

At the time of the inspection 7 children were missing the emergency contact address from their enrollment documents.

At the time of the inspection 1 child's record did not have the child's address or parent's name on the enrollment document.

At the time of the inspection a 6 month old child was present in the lunch room being supervised by caregivers who were supervising a group of children ages 2 years to 8 years of age. The 6 month old child was picked up before the inspector left the operation.

Based on information obtained through investigation it was found the operation became aware of an allegation of inappropriate discipline by a caregiver and did not make a report to Childcare Regulation.

Children were seen jumping on a trampoline on the playground. The trampoline measured over 4'10'' in diameter.

Use zones were evaluated on the outdoor playground. There were 4 stationary play items (jungle gym, trampoline, and 2 slides) that had a use zone that measured less than 6 feet from the building or fence. Photos were taken.

There were several play items on the playground that did not have surfacing. The play items included: slides, a jungle gym, and a trampoline. The play items were resting on grassy area. Photos were taken.

One child's file did not have verification of immunzations.

Two employees did not have verification of TB tests.

The center did not have verification of a health inspection.

The playgound only had one exit gate.

The written operational policies did not address promotion of indoor and outdoor physical activity that is consistent with the Minimum Standards.

The light fixture cover in the Orange Room needs to be replaced. Doors to the restrooms were leaning on the walls and need to be installed. A mirror in the restroom needs to be installed. Photographs were taken of the deficiencies.
