A second caregiver was missing annual training for training year 1/24/2024 - 1/23/2025 on the following topics: (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Administering medication; (4) Preventing and responding to emergencies due to food or an allergic reaction; (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6) Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?). A third caregiver was missing annual training for training year 1/2/2024 - 1/1/2025 on the following topics: (1) Administering medication; (2) Preventing and responding to emergencies due to food or an allergic reaction; (3) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (4) Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

The diaper changing mat in one of the infant rooms was torn. This created the diaper changing mat to become absorbent and hard to clean.

A caregiver did not have current training on pediatric cardiopulmonary resuscitation (CPR). The certificate available for review had a renewal date of 3/12/2025.

A caregiver did not have current training in pediatric first aid with rescue breathing and choking. The certificate available for reivew had a renewal date of 3/12/2025.

A wooden play structure was observed with chipping paint in the pre-k/School-Aged outdoor playground. Chipping paint was observed on several walls in one of two toddler rooms. This posed a safety/health hazard as the areas with chipping paint were accessible to children.

Based on the information obtained during the investigation a caregiver did not wash children's hands with soap and water after outdoor activities in the toddler room.

Based on the information obtained during the investigation, it was determined a caregiver used inappropriate treatment of a child by restraining a child on multiple occasions. Which prevented the child?s ability to crawl away from caregiver.

Based on the information obtained during the investigation, it was determined a caregiver did not demonstrate good judgment when the caregiver roughly picked up a child by the arms on multiple occasions.

Based on the information obtained during the investigation, it was determined that the operation did not notify Child Care Regulation about an incident which involved a child at risk for possible injury or harm by a caregiver.

One out of four staff was missing training for communicable diseases, administering medication, building and premises safety, hazardous materials, and emergency preparedness.

The emergency preparedness plan located in the parental handbook did not address the items required in 746.5202 (1-5). The information that was missing included a physical address, how children with limited mobility or who are under 24 months will be evacuated, how staff will continue to care for children until parents arrive, how staff will communicate with parents, the information that needs to be taken with them in the event of an emergency, and evacuation, shelter in place, and lock down procedures.

Based on the information obtained during the course of the investigation, staff used highchairs to restrain children as a form of discipline. Multiple children were observed in high chairs for a minimum of 18 minutes to approximately 2 hours. Children were also observed being taken out of a highchair and then put back into a different highchair while other children were napping. One staff was also observed using an inappropriate tone when speaking to children. One staff was observed in the classroom playing with children and was not interacting with two children/infants in highchairs.

Based on the information provided during the course of the investigation, one caregiver was viewed on video footage taking toys away from children and throwing them across the room.

A 17 month old did not have their diaper changed between the hours of 10am-2:30pm.

Based on the information provided during the course of the investigation, one caregiver was observed on video footage leaving a classroom with infants/toddlers up to five times within an eight-minute period while children were napping.

The operation failed to report an incident in which children were left in highchairs for disciplinary reasons. The operation also failed to report that one 4 yr. old child was left outside unsupervised for approximately 4 minutes.

A review of the background check information for an employee showed that personal information contained an email that did not belong to the employee and resulted in disclosure of personal information to an incorrect email address.

In the toddler classroom, the children were observed to be required to stay inactive at the table for two seperate time frames, ranging from 5 to 15 minutes.

In three classrooms, the caregivers did not wash the children's hands after changing their diapers.

Two employees did not use good judgement or self-control when an inappropriate confrontation was conducted in the presence of children, exposing them to inappropriate behavior.

Two caregivers at the operation used prohibited forms of punishment by grabbing and pulling a child's arm, swatting a child and yelling while redirecting and disciplining two children.

A parent was not notified of their child's injury, until after the operation's staff was questioned for information.

A caregiver was observed in two videos, covering a toddler's head with a sheet, while putting the child to sleep during nap time.

A complete food allergy emergency plan was not on file at the operation for a child with a known food allergy.

The operation served foods containing dairy to a child with a diagnosed food allergy.

The front entrance door was broken, would lock and not open properly. NOTE: This was corrected at time of inspection.

In the infant classroom, a 4 month old infant was observed asleep in a restrictive device.

A caregiver at the operation used a prohibited form of punishment by slapping the back of a child's head.

The director did not notify Childcare Licensing about an incident that involved harm to a child and a situation that placed children at risk of harm.

Once the operation's director became aware that a caregiver mistreated a child, the parent was not notified.

The operation's director did not notify the parents that the children were present during a hazardous confrontation between a caregiver and a parent.

In the toddler classroom, two separate groups of toys, were observed accessible to children and pose as a choking hazard.

Two staff working with children under the age of two years old, did not have current trainings for Shaken Baby Syndrome, Sudden Infant Death Sydrome and Brain Development.

One caregiver did not have current CPR and First Aid training.

During the DFPS investigation, it was determined that the infant, toddler, and pre-k classrooms were out of ratio.

At time of inspection, there were two classrooms in which caregiver's personal belongings were stored in an unsecured cabinet and accessible to children. There was one classroom that had a spray bottle of bleach water on top of counter next to children's handwashing sink; this bottle was accessible to children in care. NOTE: CORRECTIONS WERE MADE AT TIME OF INSPECTION.

At time of inspection, operation did not have a current sanitation inspection on file. The last inspection was dated 4/1/2021. Director has scheduled new inspection.

One toddler was not appropriately supervised when caregivers were unaware of the child's presence on the playground.

Based on the DFPS investigation, the director transitioned four additional children to the outside playground area without informing her two caregivers that the children were added to the group.

Based on the information provided during the course of the investigation, one staff told children they would not get cookies or Cheetos if they did not take a nap. Staff member was also observed clapping at the children to get their attention or getting in their face as well as roughly placing children in chairs and then using her leg to kick the chair under the table. During the investigation inspection, Inspector also observed the staff member kicking toys while in the classroom.

Based on the information provided during the course of the investigation, one staff used poor judgment when on the phone for approximately 20 minutes while caring for children.

Based on the information provided during the course of the investigation, one staff member was observed not properly supervising children while children were in care.

One out of five staff required pre-service and was missing 7 hours of training. The pre-service training was not completed within 90 days of employment. That staff needed training on food allergies, building and premises safety, and emergency preparedness.

One out of five staff had the required annual hours but was missing training on food allergies, disposing of hazardous materials, and building and premises safety. One out of five staff was missing 10 hours of training and did not have food allergies, emergency preparedness, preventing of communicable diseases, and building and premises safety training on file. One out of five staff did not have building and premises training on file.

The director's First Aid/CPR expired in April 2021.

The operation did not have documentation of fire drills being conducted. The last documented drill is from July 2020.

The operation did not have documentation between 2019-2021 that the carbon monoxide detector, fire, or smoke alarm are checked monthly.

Inspector observed one 5 month old infant with a blanket close to his face.

The operation allowed one person who is contracted to be present before submitting or receiving the results of an initial background check. Person was observed in the facility during the time of inspection.

The operation failed to notify Licensing on two different occasions when an infant required medical treatment and was sent to the hospital.

Based on information provided during the course of the investigation, one staff member left an infant alone and unsupervised in a classroom for approximately one minute.

One staff used poor judgment when she pulled an infant by their upper arm to move them away. The caregiver also used her foot to push the child forward causing the infant to hit their face on the floor. The staff member also used poor judgment by placing a towel in a crib with an infant while the infant was on their stomach, a towel with an infant in a bouncer, and a towel with an infant in a swing. Based on information provided during the course of the investigation, the caregiver also used poor judgment by allowing one infant to sleep in a swing for a significant amount of time.
