In the playground area there were 2 hazards seen, a broken soccer goal, and a small broken Basketball hoop. NOTE: This was corrected at inspection when both soccer goal and basketball hoop were removed from the playground.

One out of the four employee records reviewed is missing several of the required trainings for the annual year 6/5/2023-6/5/2024. The missing trainings include at least one clock hour in the following areas: prevention, recognition, and reporting of child maltreatment; emergency preparedness; preventing and responding to emergencies due to food or allergic reactions; recognizing and preventing shaken baby syndrome and abusive head trauma; understanding and using safe sleep practices to prevent sudden infant death syndrome (SIDS); and understanding early childhood brain development.

Infants and children older than 18 months were combined for nap time but the youngest was 4 months old and the oldest was 3 years old. Resulting in the oldest child being more than 18 months older than the youngest child. Corrected during inspection by moving the older children to another room.

Emergency medical transport and treatment authorization forms for each child being transported was not in the vehcile used to transport the children.

In the infant room there was an accordion safety gate in the doorway to enter the infant classroom, accordion safety gates are prohibited equipment for infants. NOTE: This was corrected at inspection when caregiver removed the safety gate from the doorway and put it away in a storage room.

In the outdoor area 2 water tables were not maintained in a sanitary manner.

Three garbage cans were seen without a lid making it accessible to children.

Two ointments were observed to not be labeled with a child's name. NOTE: This was corrected at inspection when caregiver removed the ointments from the diaper changing area and disposed of them.

Operational policies were reviewed and were missing 3 sections regarding promotion of physical activity(18), Safe sleep policy for infants(9), and Procedures for providing and applying sunscreen and insect repellent(19).
