The restrooms in Preschool 1 and Preschool 2 classrooms had locks on the door handles that children could reach.

An exit door in Toddler Rm 2 was blocked by stacked child chairs resulting in only one available emergency exit in the room. NOTE: The chairs were moved during the inspection.

One child's vaccination record has not been updated and is missing two vaccines. NOTE: This is a re-cite.

The transportation binders utilized to transport children did not contain copies of emergency medical authorization forms.

Six out of ten child files evaluated were missing updated vaccination records.

One out of three vehicles utilized by the operation had four frayed seatbelts.

The metal slide and monkey bars did not have any additional use zone surfacing added. NOTE: The blue spinner had been removed from the playground.

Room 3 had an electrical cord from a fan mounted on the wall accessible to the children. There was also a black lamp cord hanging from a shelf behind the changing table accessible to children. NOTE: All sunscreen and diaper cream bottles were observed stored in classroom restrooms on high shelves out of the reach of children.

One classroom had diaper creams and ointments accessible to children in the restroom. NOTE: Caregiver placed the container on a shelf out of the reach of children during the inspection.

The metal slide, monkey bars and blue spinner toy on the playgrounds had measurable loose-fill depths ranging from 0-4.5 inches.

Two classrooms had hanging electrical cords from mounted fans on the wall. The cords were within the reach of the children. One classroom had a container of sunscreen accessible to the children in the room. The bottom of a portion of the fence on the prek playground had a hole approximately 6.5 inches dug under the fence. NOTE: The classroom with the sunscreen container placed the items on a shelf in the restroom out of the reach of children.

Two caregivers were observed telling children "no, don't", "I am counting to 2 and you better..", meal time was over and to hurry up.

The blue choo-choo train on the prek playground had an exposed screw.

The operational policy handbook did not contain information regarding the promotion of active play to include the benefit, duration for age groups, type and required clothing.

The fence that backs up to the residential homes behind the operation has numerous areas of Morning Glory vines growing that are accessible to children.

Infant room 3 had two out of three shelves were turned against the wall limiting access to the toys.

There are two areas on the front pre-school playground where the bottom of the fence can be pushed out potentially allowing a child to climb under the fence.

Eight children, ages 12-18 mos, were observed sitting in bucket seats having snack. Safety straps were available in the seats but not secured around the children. NOTE: The caregivers put the safety straps around the children after being asked by the inspector.

On the middle playground, the mulch under the slide and monkey bars was compacted causing uneven depths of loose-fill material. There were two sit and spin toys that did not have any mulch around the equipment.
