Monthly inspection for fire extinguishers and monthly tests of carbon monoxide detector were last documented on 10/2023.

Emergency drills were last documented on October 2023.

A crawl and climb caterpillar and a climbing dome were observed in the outside playground. It did not have the required surfacing.

Six employee's records were reviewed. Two employees were missing training in the following topics: (1)Emergency preparedness; (2)Preventing and controlling the spread of communicable diseases, including immunizations; (3)Administering medication; (4)Preventing and responding to emergencies due to food or an allergic reaction; (5)Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (6)Handling, storing, and disposing of hazardous materials.

Outdoor activities are conducted once a day. Infant activity plan did not include specific daily activiites.

Infant classroom was changed to a smaller classroom. Infant capacity changed from 22 to 11. Child Care Regulation was not notifiied.

Two play yards used by children under 12 months of age, were supplemented with an additional mattress. Note: Additonal mattresses were removed during inspection.

Infant feeding preference forms were not available for review.

There were two structures with a platform of 22 and 25 inches in height, in the outdoor playground. Note: Both structures were removed from the outside playground during inspection.

Electronic smoke alarm was last tested in September 2020.

One out of six records reviewed was missing parents authorization to obtain emergency medical care.

Staff First Aid and CPR trainings expired on 7/25/2020.
