Operation at times has more than 9 children present.

Observed the 18m- 2 year old room without a handwashing sink. I spoke to the director about adding the handwashing sink.

The operation does not have a fully inclosed fence for the playground. I spoke to the director about having a enclosed fence.

During an investigation it was found that children in care under the age of two are receiving screen time.

During the investigation it was found that infants are being kept in restrictive devices for 30-45 minutes.

A child was able to get away from the operation for approximately 2 minutes and was found in the road by a bystander in the area.

It was found that a caregiver places children ages 2-3 years old in time out for 5 minutes.

The required annual training on the following topics had not been completed by 2 caregivers. The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4. preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials.

The required annual training on the following topics had not been completed by the directdor. The required training topics are 1. emergency preparedness 2. communicable diseases to include immunizations 3. administering medication 4. preventing and responding to emergencies due to food or an allergic reaction 5. building and physical premises safety and 6. handling, storing, and disposing of hazardous materials.

A 4 year old child went to the restroom unsupervised. Related children were left unsupervised in the office.

A caregiver working with children under 24 months did not have the required training on file.

The operational policies were incomplete and/or lacked all the required information, 746.501 (a) (9, 25, 26, 28). Referenced standard 746.501 (a) (1-29) (b)(1-2).

One child's record did not have the updated immunization record for the child's age.

There were two infants sleeping in restrictive devices (bouncers). This was corrected when the infants were woken up to be taken inside.

Observed 1 caregiver with 10 children ages 9 months to 18 months old. The specified age of the children are 12-17 months old which puts the ratio at 1 caregiver to 5 children. This was corrected when another caregiver came into the room.

Observed 7 infants outside on the playground and the caregiver was inside the building with 3 other infants coming outside. The caregiver could not see the children outside. This was corrected when the caregiver went into the playground area.

During an investigation, it was found that the operation staff interfered with the investigation.

The operation has not had an annual fire inspection conducted in the past twelve months. The operation scheduled a fire inspection with the Bryan Fire Marshal for July 30, 2021.
