During the inspection, the operational policies were viewed and the following information was missing: (11) Immunization requirements for children, including tuberculosis screening and testing if required by your regional Texas Department of State Health Services or local health authority (20) Parent rights that are consistent with the rules in Division 5 of this subchapter (relating to Parent Rights) (23) Instructions on how a parent may access the [Medium]: (A) Minimum standards online; (B) Texas Abuse and Neglect Hotline; and (C) HHSC website. (24) Your emergency preparedness plan (28) Information on vaccine-preventable diseases for employees, unless your center is in the home of the permit holder. The policy must address the requirements outlined in ?746.3611 of this chapter (relating to What must a policy for protecting children from vaccine-preventable diseases include?) (29) If your operation maintains and administers unassigned epinephrine auto- injectors to use when a child in care has an emergency anaphylaxis reaction, policies for maintenance, administration, and disposal of unassigned epinephrine auto-injectors that comply with the unassigned epinephrine auto- injector requirements set by the Texas Department of State Health Services, as specified in 25 TAC Chapter 40, Subchapter C (relating to Epinephrine Auto- Injector Policies in Youth Facilities) and in Texas Health and Safety Code ?773.0145. (30) (b)You must also inform the parents that any area within 1,000 feet of a child-care center is a gang-free zone, where criminal offenses related to organized criminal activity are subject to a harsher penalty under the Texas Penal Code. You may inform the parents by: [Low] (A) Providing this information in the operational policies; (B) Distributing the information in writing to the parents; or (C) Informing the parents verbally as part of an individual or group parent orientation.

During the inspection, the director file was viewed and had no annual training hours for the calendar year 2024-2025.

During the inspection, there were 6 out of 10 files viewed that did not have their required 24 hours of traning for the calendar year 2024-2025. There were 2 staff with 16.5 annual training hours, 1 staff with 20 annual training hours, 1 staff with 19 annual training hours, 1 staff with 11 annual training hours and 1 staff with 12 annual training hours.

During the inspection, the transportation binder did not have emergency medical information available for review for the children being transported.

During the inspection, 10 children files viewed did not have emergency medical informaiton available for review.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The director file was reviewed and first aid had expired on 11/2024.

Director file was reviwed and CPR had expired 11/2024.

A child was dropped off at 9am and diaper wasn't changed until 12:45pm.

During the inspection, I observed disinfect wipes accessible to children in care.

The black plastic playground border on the school age playground is cracked and missing a bolt.

The annual fire inspection has not been conducted. The last inspection was conducted on 11/1/2022.

Inappropriate discipline was used with a child in care when an employee grabbed a child's hand hard enough to bruise the child's hand. Note: The employee was terminated immediately.

Two caregivers did not know the ages of the students in the toddler classroom.

One infant was observed in a chair without the required straps. Note: This was corrected during inspection.

Based on the evidence obtained during the investigation, the caregivers did not maintain supervision in the toddler classroom.

Based on the information obtained during the investigation, there was sufficient evidence to indicate parents were not notified of injuries received.

One student record was not available for review.

Based on external documentation, the identify of a parent was not recorded during pick up.

Based on the information obtained during the investigation, there was sufficient evidence to indicate the toddler classroom was out of ratio.

Based on the evidence obtained during the investigation, the caregivers did not maintain supervision allowing a child to be bitten.

Based on external documentation, two caregivers did not receive training in biting to meet the needs of the classroom with biters.

Based on the evidence obtained during the investigation, curriculum is not followed in the toddler classroom.

One caregiver was allowed to have direct access to the children in care before a complete background check eligibility was received.

One student record was not available for review.

One caregiver did not have the abuse/neglect training signed.

One student in the threes class was observed not washing hands after using restroom.

One caregiver did not have proof of education requirements.

Based on the evidence obtained during the investigation a caregiver used inappropriate discipline in the classroom.

Based on the evidence obtained during the investigation, parents were not notified of inappropriate discipline in the classroom.

The changing pad located in the twos classroom was observed torn.

One infant did not have the required infant feeding instructions.

One caregiver was observed making a bottle next to the diaper changing pad.

One caregiver did not have the required in person First Aid/CPR training.

There was one caregiver caring for 16 children in the morning drop off room. The specified age group was three years old, which makes the room over ratio by one child.

Based on the information obtained the caregivers are not demonstrating competency, good judgment, and self-control while placing students to sleep.

Based on information received there is a sufficient amount of evidence to suggest that a child was forced to sleep during naptime.

A caregiver in the toddler room, who was caring for children, did not have an eligible result from CBCU.

Based on the evidence obtained during the investigation, updated lesson plans were not posted in the toddler and preschool classroom.

Based on the information obtained during the investigation, an ineligible contractor was allowed at the operation for maintenance work on March 9th.

The infant classroom was lacking an adult sized chair.

Based on the information obtained during the investigation, caregivers in the infant classroom were using the sink to wash bottles and hands.

Based on the information obtained during the investigation, caregivers are not changing students in a timely manner.

One contractor was allowed at the operation before a complete background check eligibility was received.

The toddler mats were observed to be lacking labels.

The base board in the school age classroom was peeling off of the wall on the left and right side of the room. A wall plug was also observed to be hanging off of the wall. SN: Pictures were taken.

The toilet in the two's and three classrooms contained excrement on the toilet. The wall in the two's classroom had excrement smeared on the wall.

The school age playground was observed to have rust and defects. SN: Pictures were taken.

Two caregivers in the toddler classrooms did not know an accurate age of children they were responsible for.
