Child records were reviewed during the inspection. Three records were incomplete and missing required health and safety information.

The following information needs to be added to the emergency preparedness plan: 1. How children will be evacuated or relocated to the designated safe area or alternate shelter, including specific procedures for evacuating or relocating children who are under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing impairments. 2. How children in attendance at the time of the emergency will be accounted for at the designated safe area or alternate shelter. 3. How you will communicate with local authorities (such as fire, law enforcement, emergency medical services, health department), parents, and us. 4. How you will evacuate and relocate with the essential documentation including: (A) Parent and emergency contact telephone numbers for each child in care; (B) Authorization for emergency care for each child in care; and (C )The attendance record information for children in care at the time of theemergency; 5. How you will continue to care for the children until each child has been released. 6. How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

A child was missing their food allergy plan on file.

A child was missing information of persons other than a parent to whom the child may be released.

All four of the children's files reviewed were missing heathcare provider statements.

A child was missing parents acknowledgment that they received the operation's policies.

A child was missing information for an emergency contact.

An employee file reviewed was missing documentation they received irientation.

A child was missing the appropriate authorization to obtain emergency medical care.

Operational policies were found to be missing the promotion of physical activity standard A-C.

A volunteer who has been here more than 10 consecutive times has not meet the 15 hours of training requiremnts. It was unknown that it was needed.
