One caregiver did not have an active background check during the time of inspection. The caregiver was sent home and will not return until her background is active in the system.

The operation failed to submit an updated copy of the emergency preparedness plan. This is a re-cite. The emergency preparedness plan on file did not have information that discusses the physical address of relocation, how children with limited mobility will be evacuated, and how children will be cared for until parents arrive.

The operation failed to submit an updated copy of the operational policies for medication. This is a re-cite. The operational policies state that staff do not administer medication. However, inspector observed medication at the operation.

On out of three staff was missing child maltreatment training, communicable diseases including immunizations training, administering medication, food allergies building and premises safety training, and emergency preparedness training. One out of three staff was missing training in communicable diseases, administering medication, food allergies, building and premises safety, and emergency preparedness. One out of three staff was missing administrating medication training.

Based on the information provided during the course of the investigation, one staff used inappropriate discipline when the caregiver yanked a child's arm while picking the child up from the floor. The caregiver then proceeded to place the child over their shoulder to carry them.

The operation removed the director in charge in October 2021 and has not submitted the required paperwork for a new director at the operation. The operation currently does not have a program or site director for this facility.

Based on the information obtained during the course of the investigation, staff at the operation used poor judgment when children were allowed to sit on their lap. Staff were also observed placing a child over their shoulder to carry them.
