The director and two other employees that are designated drivers did not have current transportation safety training.

The director and one caregiver that transports children only obtained one clock hour of transportation safety training. They each will need to obtain one additional clock hour of training.

One caregiver did not have prevention, recognition, and reporting of child maltreatmen training.

The director will need to complete the following trainings: *Emergency preparedness *Prevening and controlling the spread of communicable diseases

One child's record lacked the name, address, and telephone number of the child's physician or an emergency-care facility.

One child did not have an updated immunization record on file.

Two staff files lacked educational documentation.

Three children did not have a health statement on file.

One caregiver did not have current first aid training available for review.

The director completed CPR training but the training was completed fully online. CPR training must consist of a curriculum that incudes use of a CPR manikin and both written and hands-on skill-based instruction, practice, and testing.

The director and 3 staff that transport children completed 1 clock hour of transportation safety training. Two hours of annual transportation safety training is required. Each will need to complete an additional 1 clock hour of training.

Three caregivers did not have the following trainings on file: *Administering Medication *Preventing and responding to emergencies due to food or an allergic reaction *Handling, storing & disposing of hazardous materials

The director did not have current SIDS, shaken baby, & early brain developoment training.

The director and 3 staff that transport children did not have current transportation safety training on file.

One of the vehicles used to transport children did not have a fire extinguisher.

The director did not have current cpr/first aid training. The training was expired.

One vehicle purchased after 12/31/13 was not equipped with a child safety alarm. The operation will need to have a safety alarm installed.

The director did not have the following trainings on file: *Administering Medication *Preventing and responding to emergencies due to food or an allergic reaction *Handling, storing & disposing of hazardous materials

The operation did not have a current health inspection on file. The last health inspection was completed 9/22/20.

The operation failed to report two confirmed cases of a communicable disease to Child Care Regulation.
