Based on a DFPS investigation, the operation failed to report to the Texas Abuse and Neglect Hotline or Child Care Regulation regarding an allegation received that a child may have been abused.

Medication kept at the operation expired in March 2025 and May 2025 for three children.

Three out of ten employee files evaluated had not completed 24 hours of annual training for the employee evaluation year reviewed.

One caregiver had an expired CPR certificate that expired 6/2025.

The last sanitation inspection report was dated 4/16/2024.

One caregiver had an expired first aid training certificate that expired 6/2025.

The operation last had a fire inspection on 5/7/2024.

A caregiver was observed sitting on top of a picnic bench on the playground using a cell phone.

A 3 year old child was left unattended in the restroom for approximately 8 minutes when the classroom exited the building and went outside to the playground.

4 out of 10 staff training records reviewed were missing training in the following topic areas: emergency preparedness; preventing and controlling the spread of communicable diseases, including immunizations; administering medication; preventing and responding to emergencies due to food or an allergic reaction; building and physical premises safety; and handling, storing, and disposing of hazardous materials.

One rear seat belt does not retract and can not securely restrain a child in the seat. NOTE: Maintenance fixed the seat belt during the inspection.

The operation had an inhaler on site for a child without an authorization form signed by the parent. The last authorization on file was from 2019. Note: The medication was removed and will be sent home with the parent at pick up.

One out of 10 staff files reviewed was missing training records. The caregiver, hired 1/23/2019, only had 13 of the required 24 training hours recorded for the year evaluated.

Three out of 10 child files reviewed were missing immunization records. One child, age 4.9 years did not have a immunization record on file. Two children, ages 4.7 years and 4.3 years, were missing their varicella vaccines.
