Diaper changing supplies were within reach in two classrooms.

Caregiver was observed not wahing her hands or children's hands after changing diapers. Children were observed not washing their hands before snack.

One caregiver was observed supervising 18 children with a specified age group of 3 years. The maximum number of children in the group can be 15.

There was a fan within children's reach in the infants' classroom. There was a knife on top of the sink in the school age classroom. There were chemicals inside an unlocked closet in the school age classroom. There was a bucket of water accessible to children in the school age classroom. Note: It was corrected at inspection.

Infant clasroom was left deficient in child/caregiver ratio when one caregiver stepped out of the classroom, leaving another caregiver to supervise 9 children. In the toddler room, one caregiver was observed to be supervising 16 children. The specified age for the children in group was 3 years. The maximum number a caregiver can supervise based on that specified age group is 15 children.

Ten children's files were reviewed. Three files were pending updated immunization records.

Two infants were observed to be sleeping in swings. Note: It was corrected at inspection.

Activity plans did not include specific daily activities. Activity plans did not include the outdoor activities.

There was a 22 month difference in age between a 1 month old and 23 month old children in the infant classroom. There were more than 13 children in care at the operation. NOTE: This was corrected at inspection as the two 23 month old and two 22 month old children were moved into another classroom under the care of another caregiver.

Child/Caregiver ratio was deficient. Two caregivers were providing care of 11 children with a Specified Age Group of 0-11 months old. NOTE: This was corrected at inspection as one child was moved to another classroom under the care of another caregiver.

Pre-service training of one caregiver and all annual trainings for two caregivers, excluding Abuse and Neglect training, were taught by Director of operation. These trainings are not valid as the center had been assessed an administrative penalty in the two years preceding the trainings.

The annual sanitation inspection was not available for review. The sanitation inspection at the operation was last completed on 8/28/2019.

Updated immunization records for 3 out of 3 children's records inspected were not available at the operation for review.

4 out of 4 cribs were not labeled with child's name/number in the infant classroom. NOTE: This was corrected during inspection as cribs were labeled.
