During the investigation monitoring. The teachers, director, and the employee with the provisional status stated that she supervises the children independently during normal operation hours of the operation.

The bathroom door did not have an out of order sign. The Pre-K classroom has a water fountain that needs cleaning and repair. The fountain did not have an out of order sign.

There were nails exposed through the wooden beams enclosing the play equipment. The wood had also splintered.

The oepration did not have a current gas inspection on file.
