After reviewing 4 child files, one did not have the health statement from the doctor.

Observed four tooth brushes touching each other. I spoke with the director about cross contamination and the director replaced the tooth brushes with new ones.

Observed Frebreeze on the back of the toilet, which children have access too. Also observed the changing table unlocked with bleach water in a bottle. This was corrected when the Febreeze was placed out of the reach of the children and caregivers were asked to lock the cabinet.

During a DFPS investigation, it was found that a caregiver hit a child on their leg for discipline. The caregiver no longer works at the operation.

During a DFPS investigation it was found that a caregiver covered a child's mouth with their hand to quiet them. The caregiver no longer works for the operation.

Both classrooms had their activity plans on their tablets but did not have it posted for this week for parents to observe.

During a DFPS investigation, it was observed that the classroom was not in control.

Observed a caregiver on their phone while caring for children. This was corrected when the caregiver put the phone away.

Observed a person with access to the children in care without a cleared background check. This was corrected when the person left the operation. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

Observed a caregiver change three children without washing their hands. This was corrected when Licensing spoke to the caregiver about when to wash their hands.

Observed 3 children having their diaper changed and not washing their hands afterwards. This was corrected when Licensing asked the caregiver to have the children wash their hands.

Observed 3 children being changed without the diaper changing station being sanitized in between each child. This was corrected when Licensing spoke to the caregiver about sanitizing the diaper changing station between changes.
