During an investigation at the facility, it was discovered through interviews with a staff member admitted to giving, a child in care, a peanut butter cup and the child has a known nut allergy.

When reviewing the child's binder, at the facility, it was discovered that the complete list of allergies was not listed on the child's binder.

A caregiver was observed making negative comment about a child's eating habits. The child has a documented history of struggles with weight, which have previously contributed to suicidal ideation and hospitalization.

The counts for four of the child?s prescribed psychotropic medications did not match the documentation in the records on multiple days.

According to statements from two caregivers, the child in care discarded her PM medications on the night of the incident and retained three pills of her prescribed bedtime medication to use at a later time. The child was not given a replacement dose for the discarded medications, nor did the caregiver retrieve and re-administer the retained pills.
