The slide on the preschool playground area use zone was not 6 feet from the border/perimeter.

The playground structure for the preschool age group contained broken boards on the slide, and nails protruding from the fence utilized as a border.

Two transportation vehicles observed did not have child safety alarms.

The outdoor area contained numerous hazards during the walkthrough. A water hose was unraveled near the pre-k play area, which poses a risk as a tripping hazard. A wired gate contained a hole in the gated area, and also a wire that was unraveled, which poses a risk to children in care. The play structure that contained wood that had protruding nails from the wooden structure.

Two children files observed did not include an updated health statement. One child's file observed was enrolled in October of 2022, while the other child enrollment date was documented as January of 2024.

Two staff files hired after 09/01/2023 did not have a pre-employment affidavit available for review. Licensing staff provided a copy of form 2912 via email for staff to complete while Licensing was present.

The annual sanitation inspection report was dated 08/18/2022.

From 7AM-8:19AM children between the ages of 2-8 were observed watching television for over an hour during meal/breakfast time. After the school-age children left for the day, the television was still on, portraying Nickelodeon cartoons. The childcare facility did not have a lesson plan, and/or activities for the age group that was observed.

Two infants were observed strapped into a 2-seater stroller upon arrival. The infants were in the stroller for approximately 30 minutes due to lack of staff.

A total of 14 children were observed in front of the television, 9 months-5 years of age. The two infants observed are 9 months and 11 months of age. The oldest child observed is 51 months older than the youngest infant in care.

Fourteen children, 9 months - 5 years of age were observed watching a television show, Bluey. The two infants observed were observed in a stroller.

Based on the information obtained from the DFPS investigation, there is evidence to support a caregiver was conducting janitorial duties during the time of the incident.

Five out seven staff records observed did not have a current CPR/First-Aid certification. CPR certificates observed for five staff were online courses.

Three out of ten children's records did not contain a start date.

Five out of ten children records did not contain the required information for their emergency contact. Three files observed included the name and contact number. The emergency contact did not include an address. One out of ten children records observed did not include an emergency contact.

There are wooden boards observed on the outside/active play area that poses a hazard to children in care. Nails were observed on the outer layer of the wood.

Three out ten children records observed did not include a health statement.

There was not six inches of loose-fill surrounding the play structures.

Ten children's medications were either no longer being used or expired. Note- This was corrected during the inspection when the medications were discarded.

One caregiver was missing annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, and Preventing and Responding to Emergencies due to Food Allergies.

One infant's feeding instructions have not been updated since 9/6/2021.

Three caregivers did not have orientation in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Preventing and Responding to Emergencies due to Food Allergies, and Administering Medications. Note- This was corrected during the inspection when the director provided orientation to the caregivers.
