One of the three children files reviewed, did not have the Health Statement completed. This was corrected during the inspection when the child s mother completed the document.

An Infant was observed asleep and covered with a blanket in his Pack~Play. This was corrected during the inspection when the Director remove the child from the Pack ~ Play.

Of the five staff files reviewed, no staff had training for preventing and responding to emergencies due to food or an allergic reaction.

In the children's classroom the ceiling tile is broken exposing the attic.

Of the two children's files reviewed, one did not have their current immunizations.

There was a rusty nail exposed on the wooden outdoor house play structure. This was corrected when the caregiver hammered the nail flush with the play structure.

Two caregivers were left in the home with nine children. The two caregivers do not have First Aid/CPR training.

Caregiver did not have any sleeping equipment for infants. Caregiver stated infants sleep in bouncers.

Three out of the twelve children did not have a cot or matt to sleep on. Three children were sleeping on beds.

Primary caregiver had no trainings available for the year 2021-2022.

One of two rooms used by children did not have a working smoke detector. There was an outline for a smoke detector on the wall but the smoke detector could not be found.
