During the investigation, it was determined that a parent was not provided an incident report regarding a situation that placed their child at risk when their child was involved in an incident with another child.

Evidence obtained during the investigation proved that children under the age of 5 have been playing on the arch climber on the playground.

Two infant classrooms were observed serving multiple infants breakfast directly on the same table with no barrier between the food and the table.

Infants were observed seated in a table with bucket seats and were not strapped in with the buckles. This was corrected at inspection with the caregiver buckling the children.

Two individuals have been regularly and frequently present at the operation prior to receiving notification of eligibility. One of the individuals was the primary caregiver for a group of children.

The operation failed to follow conditions listed for a caregiver's background check by allowing the caregiver to be present in a classroom alone with four children.

An infant classroom was observed with 11 infants in the class with one caregiver. The specified age group was 12-17 months of age and therefore, the max number of children one caregiver can have is 5 children and the classroom was over by 4 children.

Of the 10 staff files reviewed, none of them had a notarized copy of the form 2985.

Based on evidence obtained during an investigation, an individual was the only caregiver for a classroom, with direct access to children, and did not have proof of current First Aid training in file.

Based on the evidence obtained a caregiver was not able to intervene timely when 2 children left the outdoor play area through a gate, and to the sidewalk before the caregiver caught up to them.

Food allergy plan was not signed by parent.

The sanatation inspection was due on 7-8-2023.

The building's fire inspection was due 8-19-22.
