During the investigation, it was found that one class of infant and toddler aged children were served snack on the floor of their classroom.

In one of the toddler rooms, there was a bin of diaper creams that was stored within the reach of children. This was corrected during the inspection when the bin was moved to a shelf out of the children's reach.

There were no papertowels for children to dry their hands after handwashing. Children requested from teacher papertowels and they were later supplied after handwashing was completed and CCR addressed lack of.

Caregiver spanked a child on the hand in the classroom. This child was a family friend. Video was observed of the spank. Caregiver was immediately terminated.

There was an open drain pipe on the floor in the children's bathroom with sharp exposed jagged edges.

There was an infant asleep in a bouncer. This was corrected during the inspection when the infant was removed from the bouncer and placed in a crib.

Observed a child eatting a poptart in the bathroom. The pop tart was laying on the wrapper on the floor while other children were being changed and using the restroom. This was corrected when the pop tart was taken out of the bathroom.

One caregiver stated that there were 10 children in the classroom, but their were 11 children in the classroom.

A caregiver did not relate to a child with patience when the caregiver picked the child up from behind, grabbing the child's upper arms to move the child to another location in the room.

Observed a trashcan without a lid. The trashcan had a dirty diaper in it. This was corrected when the lid was replaced.

Observed a plastic oven with the oven lid broken exposing sharp plasic pieces where the children can be injured. This was corrected when the oven door was removed.

Observed cleaning chemicals under an unlocked sink in the school age room. This was corrected when the cabinet was locked.

Electrical outlets were exposed and accessible to children in a pre-k classroom. Safety covers were immediately placed over the electrical outlets when addressed, bringing the facility into compliance.
