An infant was observed engaged in screen time.

The infant caregiver did not have SIDS or shaken baby syndrome training.

There were three water tables with standing water observed on the playground. This was corrected at the inspection when the caregiver emptied the standing water.

There was a bottle of cleaning solution accessible to children on the changing table in classroom one. This was corrected at the inspection when the caregiver placed the bottle out of the reach of the children.

The crib mat in use with the crib is not firm and there are no tight fitted sheets.

The diaper changing table in classroom one was observed to have holes in the material.

During the inspection it was found that the annual fire inspection has not been conducted since 05/18/2022.

6 of the 10 children records reviewed did not contain health statements.

4 out of the 4 staff records reviewed did not contain the original training certificates.

A child did not wash their hands after receiving a diaper change.

During the inspection, there was dust build up on the vents and fans of the facility.

During the inspection there were 3 plugs in the room with children aged 2-3 years that did not contain a safety cover. This was corrected at the inspection when the caregiver placed safety covers in the outlets.

A caregiver did not wash their hands after changing a child's diaper.

Observed a dirty pull up in a trashcan in the bathroom that did not have a covered lid. This was corrected when the pull up was taken out of the trashcan without the lid.

The Director was missing three out of the five required training content courses.

Out of ten children's records reviewed, four records were missing complete immunizations.

The operational policies were missing several required sections.

Out of ten staff records reviewed, one caregiver was missing three out of the five required content training courses.

During the course of the investigation, it was found a child in care sustained injuries caused by two caregivers.

Based on information obtained through an investigation, there was sufficient evidence to find a child was abused while in care.

Use zones were observed to be overlapping.

Out of ten children's records reviewed, four records were missing complete immunizations.

The Director was missing three out of the five required training content courses.

Outlets were observed to be uncovered in classrooms with children younger than five years of age. This was corrected at inspection, as the outlets were covered.

Out of ten staff records reviewed, one caregiver was missing three out of the five required content training courses.

The operation did not have six inches of loose fill in the required play areas.

A five month old infant was observed sleeping on the floor on a Boppy pillow with a blanket and a soft plush toy. This was corrected at inspection, as the infant was no longer sleeping on the Boppy pillow.

Based on interviews conducted with operation staff, collateral parent contacts, and an inspection at the operation it was found the operation denied parents entry inside the building to observe their children in care.

During the inspection a child was observed walking from one classroom to another classroom without adult supervision. This was corrected at inspection, the director placed the child back in thier classroom and addressed the issue with the caregiver. The director will ensure Positive Guidance and Discipline, and supervision training will be provided and complete by each caregiver by 06/21/2021

During the walk through inspection the infant cribs were not labeled.

During the walk through insepction an infant and toddler caregiver did not wash thier hands after changing a childs diaper

During the walk through inspection the following was observed in the toddler room: a toddler was observed standing on top of a table and climbing underneath the changing table. The toddlers were observed running around the classroom not participating in the activity listed on the daily schedule.

During the walk through insepction the infant room was observed to only have 4 out of 10 cribs for the non walking infants younger than 12 months.

A toddler caregiver was observed to change a toddler diaper and place another child on the changing table before sanitizing it.

During the walk through insepction the infant classroom did not have a written activity plan

During the walk through inspection three classrooms were observed out of ratio: 1. The infant classroom child/caregiver ratio was 1 caregiver to 5 children 0-11 months of age, the classroom was observed to be 1 infant over the ratio 2. The toddler classroom was observed to have 1 caregiver to 18 children, the specified age group was 2, this classroom was over by 7 children 3. The Pre-K classroom was observed to have 1 caregiver to 28 children, the specified age group was 4 years old, this classroom was over by 10 children

During the inspection children jackets were observed stacked on top of one another and not placed in each child individual storage. This was corrected at inspection, all jacket was placed in each child's individual storage area.

During the walk through inspection a bleach spray bottle was observed on a low shelf accessible to children in care. This was immediately corrected at inspection, the bleach bottle was moved to a high shelf inaccessible to children in care.
