Two personnel records reviewed did not have the following training complete: Emergency preparedness; Preventing and controlling the spread of communicable diseases, including immunizations, Preventing and responding to emergencies due to food or an allergic reaction, Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).

One child's record reviewed did not have the latest immunization record on file available for review at the time of inspection.

One plastic playhouse was observed to have a bee hive attached to the inside of the roof where children have access to it posing a safety hazard.
