The walls in each room was observed and were dirty. The boy's bathroom toilets and urinals had not been cleaned or flushed. There were smells of old urine in the bathroom. Spots on the walls in three of the rooms had peeling paint.

Personnel and training records had not been maintained. The files were incomplete for signatures and submission.

The operation did not have a copy of the annual health inspection for review.

One of the files evaluated did not have the required high school diploma, photo id, background verification and affidavits.

There was no copy of the annual fire inspection for review.

Electrical cords coming from an electrical surge protector was exposed and accessible to the children. This was corrected when the caregiver removed the surge protector and covered the wall plug with a cover.
