Based on information gathered during the course of the investigation, the operation did not report an allegation of inappropriate discipline that was observed at the operation.

Based on information gathered during the course of the investigation, it is determined that an employee disciplined their child while in care and did not follow the discipline and guidance policy.

Based on information gathered during the investigation, it was determined that a caregiver used their hand to hit a child on their bottom who was not following directions, while in care.

The operation has multiple medications on file that are expired or no longer in use.

The operations has two outdoor play structures that have signs of wear and tear i.e, broken material that is sharp and/or rough.

The operation does not have documentation that carbon monoxide on the first level of the building has been tested since December of 2024. CI: The Carbon monoxide was tested during inspection and dates that other inspections were done was added to checklist during inspection.

During the inspection nine children were observed watching television in the cafeteria while eating.

During the inspection a caregiver was observed preparing meals in the kitchen well supervising children.

Eight booster seats were observed to be expired during inspection. CI: The booster seats were discard during inspection.

The operation has seven employees that were hired after 2023 that does not have the required affidavit.

Three children with food allergies do not have required food allergy form.

A subject with conditions was observed caring for children alone during inspection. CI: A caregiver was added to the classroom during inspection.

The operation's addmission form has not been updated to reflect the updates for the 2023 addmission form.

The Children's Product form has not been updated since 6/1/20.

The director did not notify licensing when a child was allegedly placed at risk at the operation.

Two staff are missing the annual trainings in recognizing and preventing shaken baby syndrome and abusive head trauma, understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS), and understanding early childhood brain development.

Four staff are missing the annual training in preventing and responding to emergencies due to food allergies and handling, storing, and disposing of hazardous material.

The fence on the older childrens playground has two areas, where the bottom of the fence is starting to lift upward which can easily be lifted causing an opening.
